Treatment outcomes improve when a holistic approach is taken with a client. It’s more common though, for the mind and body to be treated separately with no integration. With new pay for performance reimbursement models on the horizon, providers who embrace an accountable care delivery strategy will be the ones to succeed. The ultimate goal of this application is to provide team-based integrated care in order to better treat mind and body. With this solution, those in the behavioral health, substance abuse, social service and public health communities can share medical data with a client’s primary care physician and vice versa, enabling you to understand and consider the full medical treatment of your client.
The Primary Care Application works with Netsmart’s myAvatar™, myEvolv™ and TIER™. Within the Primary Care module you can document chief and secondary complaints. As the documentation is completed following evidence-based practice procedures (treatment recommendations) and medications are provided as a checklist recommendation for the clinician to select, you can document family histories, collect vitals, update allergies and easily do a review of systems.
In addition, the Primary Care module is available as a mobile extension to your current Netsmart enterprise applications, allowing you to use it when and where you need it. It also includes a full primary care clinical workflow:
